			<content:encoded><![CDATA[<p>Nice little review of the Transformer Prime (TFP) hardware at <a href="http://gigaom.com/mobile/asus-transformer-prime-video/">GigaOM</a>. Make sure to check the game play at around 7:55 in the video. The reviewer plugs an Xbox 360 controller into the USB slot on the keyboard dock and uses it to play Shadow Run. How cool is that.&#160; </p>
<p>I really think the hybrid design of the TFP is ideal for many situations, especially for those people that truly want to carry a single device. As much as I like tablets I find that I still need a keyboard for any significant data entry chores, whether it be with a spreadsheet, word processor, etc. </p>
<p>I would really like to see tablet PC manufacturers like Lenovo and Samsung do something similar, i.e. a keyboard dock that increases battery life and folds into a laptop style portfolio with the tablet docked. My dream machine would be a <a href="http://www.samsung.com/us/computer/tablet-pcs/">Samsung Series 7 Slate</a> with a laptop dock similar to the TFP. It doesn’t appear that Samsung is interested in such a docking solution, but I’m hopeful that a third party will take the hint and do it anyway. </p>

I mean we have real-time patient monitoring, near field communication, telemedicine, smartphones, music and video in the cloud, and so on <em>ad infinitum</em>. So why is it that hospital pharmacies use the same old horizontal hoods that they’ve always used? </span></p>
<p><span id="more-6249"></span></p>
<p><span style="font-size: small;">I’ve said it many times before, </span><span style="font-size: small;">the IV room (a.k.a. the cleanroom) in hospital pharmacies is in need of a major overhaul. Sure, we went through the big <em>USP &lt;797&gt;</em> thing a few years ago, but we didn’t really <em>change</em> anything inside the IV room. </span></p>
<p><span style="font-size: small;">After following the stream of cool technology coming out of CES 2012 a couple of weeks ago I’ve decided that new IV hoods need to make use of the following:</span></p>
<p><span style="font-size: small;">- Continuous data collection. An IV hood should be smart. It should collect information in real time; air quality, flow, etc, and provide feedback to the user as well as anyone else that needs to monitor these things. </span></p>
<p><span style="font-size: small;">- IV hoods need to have a heads-up display made from Samsung’s Transparent Smart Window. Basically it’s a full blown Windows 7 machine built into a transparent LCD screen; pretty stinking cool. It could be used to view patient information like allergies, current medication regimen, labs, etc. In addition the Smart Window could offer the person in the hood feedback on what they’re making. Two-way video communication with a pharmacist outside the IV room? No problem. Or you could just use it to check your social media.</span></p>

<p><span style="font-size: small;">- The surface of the hood should be something similar to <a href="http://blogs.msdn.com/b/healthblog/archive/2011/01/06/introducing-microsoft-surface-2-0-our-vision-for-healthcare.aspx">Microsoft Surface</a>. As things are placed inside the hood the surface could collect and analyze information on the item, including what it is (barcode, RFID, NFC), whether or not it’s compatible with other things in the hood, if air flow is blocked, check the items against the patient for whom the compounded is being made via clinical decision support tools, and so on. Real-time clinical and operational information. </span></p>

From the study abstract in <em>Langmuir</em>:</em></font></p>
<blockquote><p><em><font size="3">Here we focus on creating dispersion of highly hydrophobic vitamin E aggregates in the lenses as barriers for drug diffusion for increasing the release durations. This approach has been shown previously to be successful in extending the release durations for some common hydrophilic ophthalmic drugs. The topical anesthetic drugs considered here (lidocaine, bupivacaine, and tetracaine) are hydrophilic at physiologic pH due to the charge, and so these cannot partition into the vitamin E barriers. However, these surface active drug molecules adsorb on the surface of the vitamin E barriers and diffuse along the surface, leading to only a small decrease in the effective diffusivity compared to non-surface-active hydrophilic drugs. The drug adsorption can be described by the Langmuir isotherm, and measurements of surface coverage of the drugs on the vitamin E provide an estimate of the available surface area of vitamin E, which can then be utilized to estimate the size of the aggregates. A diffusion controlled transport model that includes surface diffusion along the vitamin E aggregates and diffusion in the gel fit the transport data well. In conclusion, the vitamin E loaded silicone contact lens can provide continuous anesthetics release for about 1–7 days, depending on the method of drug loading in the lenses, and thus could be very useful for postoperative pain control after corneal surgery such as the photorefractive keratectomy (PRK) procedure for vision correction.</font></em></p>

<p><font size="3">For those of you that haven’t experience “4G” you’re missing out. It’s quite snappy. I frequently use my Galaxy Nexus to watch movies on Netflix while waiting for my daughter at Volleyball practice. No lag. No buffering. Just a smooth movie watching experience. </font></p>

			<content:encoded><![CDATA[<p>Anyone that’s ever worked in an acute care pharmacy knows about med trays, code trays, transport boxes, intubation kits, etc. They’re a bit of a headache because all the medications inside each kit has to be manually manipulated and tracked, including the dreaded lot number and expiration date of everything in the trays. </p>
<p>Well, <a href="http://kitcheck.com/">KitCheck</a> is a system that uses RFID technology to track the medications found in all those different med trays, code boxes, etc. I thought it was pretty cool. It’s a great idea. Wish I would have thought of it. </p>

<blockquote><p><em>The KitCheck solution utilizes Radio Frequency Identification (RFID) technology to accurately determine exactly what is in a kit, what has been consumed or missing and which items are expired.</em></p>
<p><em>Pharmacies start by tagging product upon bulk receiving in the hospital. The KitCheck solution makes this process fast and painless by utilizing the bar codes present on most items currently in the pharmacy code kits.</em></p>
<p><em>Pharmacists build kits by placing items physically into the kit. When they are finished, rather than manually checking each expiration date and quantity, the user simply puts the kit into the RFID reading station for verification. The kit contents and expiry dates are validated in less than 15 seconds. Any problems with the kit contents are reported and the pharmacist can quickly correct any issues. A control log with the kit contents and expirations is automatically generated.</em></p>

<p><em>When the kit is complete, simply seal it up as you would normally. The kit is now ready to be deployed for use in the hospital.</em></p>
<p><em>After a kit is broken open for use, simply bring it back to the RFID scanning station. The KitCheck station will show you in less than 15 seconds which items have been consumed as well as which items were erroneously put into the kit. Kit use information is updated in the system to be used for patient billing or supply ordering purposes.</em>Simply rebuild the kit using the automatically generated report, and then you are ready to deploy the kit again for another use.</p>

<li><span style="font-size: small;">Biggest scandal in college football in 2011 &#8211; Take your pick: Ohio state, Syracuse, University of Miami? With that said the worst thing to hit college football in the history of the sport had to be the </span><a href="http://en.wikipedia.org/wiki/Penn_State_sex_abuse_scandal"><span style="font-size: small;">Penn State scandal</span></a><span style="font-size: small;">, hands down. </span></li>
<li><span style="font-size: small;">Worst thing in professional sports 2011 &#8211; The NBA lockout.  The NBA already has a bad reputation of catering to a bunch of spoiled, rich thugs. All the lockout did was reinforce that idea. I’m not a big fan of the NBA, and all this did was make me go from a casual fan to no fan at all. I won’t be watching an NBA game anytime soon. </span></li>
<li><span style="font-size: small;">Biggest surprise in pro sports &#8211; </span><a href="http://mlb.mlb.com/mlb/ps/y2011/index.jsp?sv=1"><span style="font-size: small;">The World Series</span></a><span style="font-size: small;">. First time in recent memory that it I felt any interest at all in watching MLB. The sport was almost dead. Perhaps this was enough to resurrect it. </span></li>
<li><span style="font-size: small;">Best in sports talk radio – </span><a href="http://espn.go.com/espnradio/show?showId=theherd"><span style="font-size: small;">The Herd</span></a><span style="font-size: small;"> with Colin Cowherd (ESPN). I think Cowherd rambles on at times, but at least he rambles on about sports. </span></li>
<li><span style="font-size: small;">Worst in sports talk radio – The Dan Patrick Show. Honorable mention Mike and Mike. </span></li>
<li><span style="font-size: small;">Best new TV series – </span><a href="http://www.usanetwork.com/series/suits/theshow/overview.html"><span style="font-size: small;">Suits</span></a></li>
<li><span style="font-size: small;">TV series that used to be good, but turned to crap in 2011 – </span><a href="http://www.usanetwork.com/series/burnnotice/"><span style="font-size: small;">Burn Notice</span></a><span style="font-size: small;"> and </span><a href="http://www.tnt.tv/series/closer/"><span style="font-size: small;">The Closer</span></a><span style="font-size: small;">. It’s the beginning of the end for both. The storyline for Burn Notice is getting old and I find myself rooting for the bad guys in The Closer because the main character has become such a self-righteous witch. </span></li>
<li><span style="font-size: small;">Best traveling companion other than my wife and family in 2011 – </span><a href="http://www.siriusxm.com/"><span style="font-size: small;">SiriusXM</span></a><span style="font-size: small;"> Satellite Radio. Why? Because the stations are always the same, no matter where I go: 84-ESPN Radio, 85-ESPN Xtra, 86-Mad Dog Radio, 88-SiriusXM NFL Radio. </span></li>

I suspect the Transformer Prime will be better, but I haven’t actually seen one to be sure. The original Transformer tablet is a great device, and the keyboard docking station only makes it that much better. </span></li>
<li><span style="font-size: small;">Best new Windows 7 Slate – </span><a href="http://www.samsung.com/us/computer/tablet-pcs/XE700T1A-A03US"><span style="font-size: small;">Samsung Series 7</span></a><span style="font-size: small;"> 11.6-inch Slate. </span></li>
<li><span style="font-size: small;">Best new rugged tablet &#8211; </span><a href="http://www.panasonic.com/business/toughpad/US/best-android-rugged-tablet.asp"><span style="font-size: small;">Panasonic Toughpad</span></a><span style="font-size: small;">. I would really like to get my hands on one of these. I like the idea of being able to use a slate tablet on the road without worrying about pampering it. </span></li>

To all you great computer makers &#8211; Lenovo, Dell, Samsung, HP – please bring the keyboard back to tablets, whether as a convertible machine or as a docking station similar to the Asus Transformer. </span></li>

E-Ink is still the best screen type for reading. </span></li>

Read it Later works better on my smartphone than Evernote. I can’t clip an entire webpage to Evernote on my smartphone, but I can send it to Read it Later. </span></li>
<li><span style="font-size: small;">Best new smartphone &#8211; </span><a href="http://www.google.com/nexus/"><span style="font-size: small;">Samsung Galaxy Nexus</span></a><span style="font-size: small;">. I have one. It’s my first new smartphone in almost three years. Suffice it to say that it is simply spectacular. </span></li>
<li><span style="font-size: small;">Technology I’d like to see manufacturers work on the most in 2012 – Battery technology. The functionality, processors, screen technology, etc has outpaced smartphone batteries. It’s an issue. </span></li>
<li><span style="font-size: small;">Favorite family trip 2011 – There were two trips that stick out in my mind this year: 1) My family and I drove to Oregon in July to attend my niece’s wedding. We spend five days near Eugene. We had a great time. 2) In early August my family and I spent two weeks running around Washington State. During that time we ventured into Idaho and Montana and took the kids into Northern Washington to visit Forks and La Push (of course we had to do this because of the Twilight books and movies). Both trips were awesome. They each generated enough good memories for a lifetime. </span></li>
<li><span style="font-size: small;">Favorite family trip 2011 honorable mention &#8211; Disney World. My daughter&#8217;s competitive cheer team qualified for &#8220;<a href="http://espnwwos.disney.go.com/events/cheer/cheer-worlds-championships">Worlds</a>&#8220;, a world-wide cheerleading championship, and it was held at Disney World in April 2011. </span></li>

This is one of the coolest, most innovative pieces of technology I’ve seen in a long time. I’ve thought about real-time liquid medication identification in the past, <a href="http://jerryfahrni.com/2010/09/real-time-infusate-verification-with-smartpumps-how-do-we-get-there/">here</a> and <a href="http://jerryfahrni.com/2011/10/witnessless-waste-a-novel-concept/">here</a>. I had the pleasure of seeing IV Check in action. Cool.</p>

<p>Regarding IV Check:</p>

IV Check can also be used to measure process streams or final product in pharmaceutical and IV manufacturing operations. It also provides a simple, one-step solution to detect diversion of narcotics, anesthetics, or other drugs, a serious problem in many hospitals. IV Check allows pharmacy technicians and pharmacists to simply and rapidly verify the drug, concentration and diluent of IVs. IV Check provides the first practical and complete verification of important medications prepared in hospital pharmacies, helping achieve a zero error workplace. IV Check is considered laboratory equipment much like an HPLC or spectrophotometer, therefore regulatory approval is not required.</em></p></blockquote>
<p>In one of my post from <a href="http://jerryfahrni.com/2010/09/real-time-infusate-verification-with-smartpumps-how-do-we-get-there/">September 2010</a> I wrote “<em>a smartpump cannot identify an intravenous (IV) medication incorrectly prepared by the pharmacy; without the rare visual queues from certain medications it’s virtually impossible. While most pharmacies have a proven track record of making perfectly safe IV products, mistakes do happen. And as unfortunate as it may be, some of these errors result in patient harm or even death in the most extreme circumstances. Several systems have been developed for use inside the acute care pharmacy to minimize IV compounding errors …. However, even with these systems in place there is nothing outside the pharmacy designed to catch an incorrectly prepared IV infusion</em>.” While IV Check won’t do what I’m looking for it appears that <a href="http://www.seamedical.com/?pg=products&amp;sl=smart-iv">Smart IV</a> by SEA Medical Systems might.</p>

The Smart IV closes an important gap in IV safety, for the first time providing the ability to intercept administration errors at the patient’s bedside. The Smart IV system consists of a pole or rack mounted monitor attached to a sensor that plugs into the IV line.</em></p>
<p><em>During IV administration to patients, the Smart IV is designed to report drug, dose, diluent, flow rate and cumulative dose instantaneously and continuously, alerting nurses to mistakes that can injure or kill patients. As IVs are changed, the monitor reports the new drug without any intervention required. A history of administrations is kept allowing healthcare providers to review the history of medications for each patient. The documentation of delivery also helps hospitals insure complete reimbursement for IV medications. This product is currently in development, will require regulatory approval in the U.S., and is expected to be launched commercially in 2013.</em></p></blockquote>
